Position Summary:

 

The discharge/transfer coordinator role’s purpose is to assist in the timely movement of patients through a set of points of care within a health care facility which compile the entire patient journey. The role involves ensuring the overall patient flow process is effective and delays are minimised to an acceptable level and within targets (HIQA KPIs) on a daily basis. The patient journey includes pre-assessment/pre-admission, admission, placement & allocation of patients to appropriate available beds in conjunction with the admissions officer/bed management teams where applicable, followed by a number of points of care incorporating a discharge plan which is planned and implemented to facilitate the transfer of a patient from the hospital to an alternative setting. This applies to both scheduled & unscheduled patients.

 

The discharge/transfer coordinator is responsible for monitoring the allocation of available beds within the NeuroCent directorate in coordination with the admissions staff/bed management. Physicians, Nursing staff, admissions staff, Health and Social Care Professionals must work in tandem to ensure gaps in discharge planning services are minimised and duplication of efforts are eradicated. He or she will be working with all disciplines throughout the organisation on maximising patient throughput and utilisation of available hospital beds and also ensuring all beds available to the hospital and in other facilities (off site) are utilised in the most efficient and effective manner. The discharge/transfer coordinator would be required to attend ward rounds to facilitate and augment decision making and to maintain a visible, accessible resource to all healthcare team members.

Key Tasks:

  • To support egress and flow of patients that are admitted in the emergency department directly to the ward.
  • To maintain an accurate and timely knowledge of bed availability using the “Visual Hospital” lean methodology. To ensure real-time patient discharge is recorded and beds assigned for patient care in accordance with hospital policies.
  • To manage the split of urgent/emergency and scheduled care to effect the daily placement of unscheduled and scheduled admissions efficiently.
  • To work with appropriate members of Interdisciplinary teams inclusive but not exhaustive of medical, health & social care professionals along with nursing staff to ensure safe admission and discharge of all patients.
  • Proactive case management of all patients discharge in accordance with the complex discharge policy, including LTC process and HSS process management to ensure efficient flow throughout the hospital.
  • Liaise closely with community, medical teams, nursing & hospital management in relation to performance indicators such as core occupancy, delayed discharges, length of stay analysis.
  • To collaborate with management information systems in collating data on the frequency of peaks and troughs of bed usage etc. in order that the hospital management can be better informed and work towards more effective bed utilisation, planning of theatre lists and other services demands.
  • To Liase with the Operational Patient Flow managers, CNMs, Senior Nurses, Clinicians, Hospital Management teams as appropriate to identify and help resolve barriers to discharge and where appropriate to identify anticipated discharge date.
  • Work in partnership with patients, their relatives, and carers to develop optimum plans for discharge providing resource information to assist with informed decision making.
  • Liase with community Care facilities, Nursing Homes and National Rehabilitation centre to facilitate the placement of patients.
  • Ensure the adequate and appropriate communication systems are in place on a multidisciplinary level facilitating effective and efficient discharge/ intra hospital transfers.
  • To liaise with the relevant members of the multidisciplinary team to ensure best possible planning and co-ordination access to diagnostic investigations and To participate in meetings and working groups on issues surrounding bed utilisation and discharge planning, representing the hospital, if required.
  • Perform such other duties appropriate to the office that may be required from time to time.
  • Ensure appropriate discharge/ transfer documentation has been generated for the management of patients awaiting discharge.
  • Review and update documentation relating to discharge planning to improve quality, accuracy and timeliness of documentation information.
  • Participate in the development of appropriate Admission and Discharge policies.
  • To play an active role in education of all staff Medical, Nursing , Allied Health Professionals, Management and Clerical Staff in relation to discharge planning, discharge and transfer hospital policies.
  • Provide training and education for clinical staff on the admission and discharge policy, practice and procedure, to improve the quality of the patient’s experience of hospital discharge, to improve patient outcomes and reduce delays in transfer of their care.
  • Initiate and participate in research and education relating to discharge.
  • Collate and analyse contributory factors and/or causes of delay in the patient’s planned discharge process and to report these to the interested parties to implement quality improvement initiatives.
  • Audit effectiveness of planned discharges.
  • Report on Key Performance Indicators (KPI’s), including performance indicators, such as lengths of stay and bed occupancy and action same.
  • To engage with Out-Patient Parenteral Antimicrobial Therapy (OPAT) and Community Intervention Team (CIT) and maintain regular communication links with all patient flow departments in other hospitals.
  • In conjunction with line manager agree learning objectives designated to meet development goals to improve service.

Decision Making

The discharge/transfer coordinator must have the ability to make decisions in a timely fashion with the best interest of the patient in mind using all their available information.

 

Analytical

The discharge/transfer coordinator will have the ability to manage and co-ordinate the placement of patients into appropriate beds with maximum efficiency and coordination. He/she will have the understanding and foresight to predict and plan necessary bed movements to ensure the best possible care for patients at all times.

 

Communication

The discharge/transfer coordinator will have exemplary communication skills that will enable him/her to communicate clearly and effectively between the many disciplines throughout the organisation. He/she will have the skill and judgement to use their communication skills to best effect in developing working relationships between all concerned. Stress Management

The discharge/transfer coordinator will have the necessary skills and abilities to handle the conflicting agendas and competing demands that are integral to the efficient management of admissions and discharges within the NeuroCent directorate and within the wider hospital.

 

Strategic Awareness

The discharge/transfer coordinator will have an understanding of the Irish health service and the effect of external forces upon the hospital in relation to admissions, discharges and length of stay. He/she will understand the difficulties of working with limited resources and the reliance by the hospital on external funding sources for continued activity.

 

Service Planning 

The discharge/transfer coordinator will understand the need for and the implementation of service planning and its effect on overall hospital activities.

 

The person will be dynamic, energetic and enthusiastic. They will have a keen desire to develop and maintain systems that promote maximum efficiency within the NeuroCent directorate and with the hospital adapting to constant change to improve the service. They will be willing to innovative and adapt as part of crisis management and they will have the skills to work together with others to preserve a team approach to the management of patient throughput into the NeuroCent directorate and in Beaumont Hospital.

 

The job description represents the main functions of the role and is subject to review and amendment to reflect the needs of the service.

Selection criteria outline the qualifications, skills, knowledge and/or experience that the successful candidate would need to demonstrate for successful discharge of the responsibilities of the post.

Mandatory Requirements:

1. Statutory Registration, Professional Qualifications, Experience, etc
(a) Eligible applicants will be those who on the closing date for the competition:

  • Are registered in the relevant division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.
  • Have at least 5 years post registration experience (or an aggregrate of 5 years fulltime post registration experience) of which 2 years must be in the speciality or related area.
  • Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
  • Candidates must demonstrate evidence of continuous professional development.

(b) Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.

 

2. Annual registration

  • On appointment, practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).
  • Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

3. Health
Candidates for and any person holding the office must be fully competent and capable of
undertaking the duties attached to the office and be in a state of health such as would indicate a
reasonable prospect of ability to render regular and efficient service.

 

4. Character
Candidates for and any person holding the office must be of good character.

 

 

Desirable Criteria:

  • Possess working knowledge of computer systems, to include spread sheets and word processing.
  • Be aware of PPPGs, policies related to Patient Flow and Discharge planning.
  • A management course is desirable.
  • Neurosurgical experience desirable but not essential
