(LICSW) Licensed Independent Clinical Social Worker

Constellation Health Service is looking for a Part-time Licensed Independent Clinical Social Worker (LICSW) for our Hospice Care Programs in Dover, New Hampshire and Newburyport, MA!

Schedule: 24 Hours Per Week Part-Time

Territory: The North Shore Massachusetts and Dover, New Hampshire

Constellation Health Services is looking for a compassionate and empathetic Licensed Indepented Clinical Social Worker (LICSW) to work with our hospice patients. This role will involve providing care in the patient's homes, assisted living facilities and in Skilled Nursing Facilities.

This Is You:

LICSW license in the state of Massachusetts and New Hampshire with at least 1 year of clinical experience; Hospice, Acute Care, and/or Bereavement Counseling preferred
Have reliable transportation and be willing to commute within a designated territory to provide the best in-home patient care.

Together, we:

Provide Clinical Social Work to our hospice population. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plans intervention based on evaluation findings
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record-keeping system. Submits all clinical documentation, time records and other information in the required time frame
