  • Supporting NDIS participants, along with their families and carers, to identify their needs through assessment and person-centred planning
  • Owning the assessment, development and delivery of evidence-based support plans within a complex support environment
  • Driving meaningful goals and measurable outcomes, using a positive behaviour support framework and a commitment to eliminating restrictive practices
  • Ensuring plans reflect each participant’s goals, strengths and what matters most to them
  • Building the capability, confidence and understanding of those supporting participants in their day-to-day lives
  • Working collaboratively within a multidisciplinary team, including mentoring less experienced practitioners

Qualifications

To be successful in this role you will have:

  • A degree qualification as an allied health professional
  • Evidence of suitability at Core, Proficient, or Advanced level.
  • Experience in report writing
  • Strong organisational skills and the ability to manage a varied caseload
  • A proactive, self-motivated approach, with a drive to improve outcomes and test new ideas
  • A genuine passion for working in the disability sector

What's in it for you?

  • Ownership and impact, manage your own caseload and see the direct difference your work makes over time
  • Great benefits including salary packaging, referral bonuses, extra leave purchase options and corporate discounts
  • True flexibility to choose the hours and days that suit your lifestyle
  • Competitive package with industry-leading salary, kilometre reimbursement, laptop, smartphone, PD allowance and performance rewards
  • Balanced work style combining work from home, community visits and access to office and clinic spaces
  • Genuine support with weekly supervision, dedicated mentors and senior clinician guidance
  • Career growth through tailored training, professional development and pathways across Ability Action Australia and MedHealth
