Case Manager - In Home Care Temp

Are you passionate about making a meaningful impact in the lives of older individuals? Edwards and Co are seeking a dedicated TCP (Transition Care Program) Case Manager to support aged care clients through their transition to care programs, develop therapeutic care plans, and ensure smooth discharge planning. In this vital role, you will manage the clinical needs of in-home aged care clients, working closely with clients, families, and healthcare professionals to provide exceptional support, responsive care, and high-quality services across the region.

This is a temp role which gives you the flexibility to work when suits you! Please note you must be local to the Townsville area or within driving distance.

Role Requirements:

• Experience within an in home/community or aged care facility setting
• Current AHPRA Registration
• Your own reliable vehicle
• Current drivers licence, car registration and insurance
• Current CPR Certificate
• Current police clearance certificate 
• Up to date with COVID-19 and Flu Vaccinations
