Edwards and Co are seeking a dedicated TCP (Transition Care Program) Case Manager to support aged care clients through their transition to care programs, develop therapeutic care plans, and ensure smooth discharge planning. In this vital role, you will manage the clinical needs of in-home aged care clients, working closely with clients, families, and healthcare professionals to provide exceptional support, responsive care, and high-quality services across the region.
