Job Description - Client Service Manager

Our client is a community nursing and support service provider who has been servicing Western NSW for over 20 years. They are looking for a passionate Client Service (Care) Manager with a genuine interest in Aged Care to provide service to their clients living in their own homes in Dubbo and the Central West NSW.

As a Client Service (Care) Manager, you will be responsible for:

Delivering person-centered co-ordination to clients

Attending consultation visits.

Completing initial intake assessments.

Developing care plans in line with their budget.

Case management and monitoring and reviewing client well-being

Requirements

Current driversâ license and reliable vehicle

Excellent communication skills

Pride in the work you do with an eye for detail and high standard of care

The ability to thrive in a close-knit team environment
