You’ll work collaboratively and proactively through telehealth to help customers stay safe and independent at home. You’ll triage clinical concerns with care, confidence, and clarity, providing expert clinical input to guide decisions.

Key Responsibilities:
Effectively deliver on all requirements of the Clinical Roster including:
- Manage the clinical Inbox and task list
- Triage the customer health and wellbeing notes for identification of, and response to, deterioration
- Completion of Validated Assessment Tools (VATs)
- Assist the other HomeMade teams via booked sessions to deliver clinical guidance and advice
- Complete referrals for Comprehensive Nursing Assessments (CNAs)
- Complete clinical reviews, formulating recommendations to promote health, safety and wellbeing for our customers, and supporting customers and their representatives to understand the recommendations and to make informed decisions to accept or decline recommendations
- Review and respond to clinical escalations, and to incidents and complaints with a clinical component
- Provide guidance and support to customers and their representatives on palliative care and advance care planning
- Provide subject matter expertise and guidance on HomeMade clinical policies and procedures to contracted service providers
- Uphold delivery of services in accordance with the Aged Care Act and Aged Care Quality Standards
- Participate in, contribute to and implement clinical quality improvement and clinical risk management initiatives, as allocated by the Clinical Manager.
- As directed by the Clinical Manager, provide clinical subject matter expertise and support to wider business.
- Maintain practice alignment with the Clinical Governance Framework at all times
- Maintain AHPRA registration and CPD requirements.
- Work with the Clinical Manager to recognise opportunities for service innovation and take a proactive approach to implementation.
- Promote a culture of best practice and shared knowledge by sharing your deep experience in the home care sector and the broader aged care system.
Your skills and experience:
- AHPRA Registration as a Registered Nurse
- First Aid certification
- Experience in Home and Community Care, ideally with aged care clients
- Strong clinical decision-making and case management skills
- A warm, human approach and genuine passion for empowering others
- Excellent communication and customer service skills
- Bonus points if you know your way around self-managed or consumer-directed care
