About the Role:

As an In-Home Withdrawal Service Nurse, you will be part of a multidisciplinary team of Nurses, Alcohol and Other Drug (AOD) Clinicians and Peer Practitioners delivering evidence-informed withdrawal support to people aged 12 years and over across Northern Adelaide and Central Adelaide.

Working within Sonder's innovative In-Home Withdrawal Service (IHWS), you will provide person-centred nursing care across all stages of the withdrawal journey, supporting clients to safely withdraw from alcohol and other drugs in the comfort of their own home. You will work closely with General Practitioners, the Clinical Nurse Lead, Peer Practitioners and external partners to ensure coordinated, safe and high-quality care.

In this role, you will:

• Conduct comprehensive clinical assessments, including withdrawal severity screening, mental and physical health assessments, and home environment risk assessments.

• Deliver evidence-informed, GP-supported withdrawal management, including daily clinical monitoring, medication management oversight and timely clinical escalation where required.

• Provide therapeutic nursing interventions, education and support to clients, families and support people throughout assessment, preparation, withdrawal management and post-withdrawal care.

• Promote harm reduction strategies, including overdose education and reduced tolerance risk management following withdrawal.

• Collaborate with internal and external stakeholders, including GPs and Drug and Alcohol Services South Australia (DASSA), to provide integrated, seamless care and strong continuity of support.

• Contribute to a positive, recovery-oriented and multidisciplinary team environment focused on achieving the best possible outcomes for clients.

The In-Home Withdrawal Service (IHWS) provides a structured, evidence-based alternative to inpatient withdrawal management, supporting people aged 12 years and over to safely withdraw from alcohol and other drugs in their own home.

The program follows a four-stage model of care:

• Intake and Brief Intervention

• Pre-Withdrawal Preparation

• In-Home Withdrawal Management

• After-Care and Continuity of Care

Through a multidisciplinary approach, the service delivers holistic, coordinated and person-centred care that promotes recovery, reduces harm and supports long-term wellbeing.

Experience Requirements:

• Minimum 4 years’ supervised experience in community and/or clinical AOD service delivery settings

• Experience delivering withdrawal management using validated withdrawal scales

• Experience working alongside lived experience / peer workforce colleagues in an integrated multidisciplinary model

• Experience conducting home visits including lone worker safety protocols

• Experience providing clinical mentoring, preceptorship or informal supervision to less experienced Registered Nurses, student nurses or multidisciplinary colleagues

Qualification and Other Requirements:

• Current unrestricted Division 1 AHPRA registration.

• Classification: appointed at Clinical Nurse (RN2 / CN) level in accordance with Sonder's clinical classification framework:

  • Registered Nurse Level 2 (RN2) - minimum four years post-registration experience, including at least two years in AOD, mental health, community nursing, justice health or emergency / acute care with AOD exposure

  • Clinical Nurse (CN)- appointment at CN level available where the candidate holds relevant postgraduate qualifications (e.g. Graduate Certificate in Alcohol and Other Drugs Nursing, Mental Health Nursing, Community Nursing, or Addiction Studies) OR demonstrates substantial specialist experience recognised through Sonder's clinical credentialling process

• Current or willing to obtain a Working with Children's Check (WWCC)

• Current or willing to obtain a National Police Check (NPC)

• Employment is subject to compliance with Sonder’s Immunisation Policy. Applicants must hold, or be willing to obtain, all vaccinations and immunisations required for the role.

• Highly desirable: Credentialed Mental Health Nurse (CMHN) status

• Highly desirable: Current training in naloxone administration and overdose response
