About the Role

Grampians Health Horsham is a 20 bed sub-acute facility that provides admitted rehabilitation, geriatric evaluation and management (GEM), transition and palliative care services within a multidisciplinary team and patient focussed environment.

• Provide clinical patient care activities in alignment with your educational preparation and level of competence


• 7-day, 24-hour rotating roster, including day, afternoon, and night shifts.


• Permanent full-time, part-time, and casual positions available


• Provide exceptional patient care

Key Responsibilities

• Deliver individualised nursing care as part of a multi-disciplinary team


• Liaise and effectively communicate with relevant medical, nursing and allied health team members


• Promote evidence-based practice that has a problem-solving approach


• Encourage patients, carers and families to participate in all stages of care


• When appropriate, supervise and direct clinical practice of junior Registered Nurses, Enrolled Nurses, and undergraduate nursing students and provide feedback

Skills & Experience

• Registered Nurse with current AHPRA registration


• Excellent communication and problem-solving skills


• Knowledge of contemporary nursing theory and practice in relation to the care of sub-acute patients/clients


• Thorough understanding of quality systems and health services accreditation

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Registered Nurse Grade 2 (YP2 - YP9) which ranges from $42.60 - $54.40 per hour and Enrolled Nurse (IB68 - IB75) which ranges from $38.13 - $44.85 per hour.
