Grade 3 Occupational Therapist - Rehab at Home

About the role:

The Senior Occupational Therapist plays a pivotal role in managing and coordinating Occupational Therapy services for patients receiving rehabilitation in Rehab at Home. Combining clinical expertise with leadership, this role focuses on improving patient outcomes while driving team development. As a senior member of the team, the position involves effective collaboration with key stakeholders during discharge planning, ensuring that realistic and achievable goals are set for patients' rehabilitation journeys. 

Key responsibilities: 

• Promote and support safe and early discharge from inpatient settings and emergency departments as well as reducing length of stay of rehabilitation patients in acute and subacute settings  


• Demonstrated specialist knowledge in clinical reasoning and discharge planning 


• Maximise patient outcomes in their own home through occupation focused rehabilitation 


• Excellent communication, negotiation, and interpersonal skills 

About you: 

• Advanced clinical expertise in Occupational Therapy 


• Proven leadership experience, including managing and delivering service teams to achieve KPIs and quality outcomes 


• Prioritise and balance demands between providing direct clinical care and engaging in non-clinical tasks such as quality improvement initiatives, program coordination, and strategic development. 


• Strategic thinking with a focus on quality improvement
