About the role

The key purpose of this role is to provide services and support to daily living that promotes independence, reablement and wellbeing of clients, to optimise their health, wellbeing and quality of life in accordance with their care plan and in collaboration with the Post Acute Care Coordinators, the client, their family and members of the health care team, while working within the scope of practice of the Health Care Worker role and delivering safe, high-quality, person-centred care.

Key Responsibilities

• Collaborate with the multidisciplinary team to achieve positive health outcomes for clients.


• Provide high-standard direct client care as outlined in care plans under the supervision of the Coordinator and Team Leader.


• Ability to Identify and escalate any changes in the clients condition or behaviour in a timely manner.


• Undertakes a range of domestic duties to support the clients nutritional, environmental and hygiene needs.


• Accurately document clients care in accordance with health service guidelines and promptly report any incidents in the VHIMS system.


• Organise workload in order to achieve planned care requirements

 Skills and experience

• Registered Drivers License 


• Certificate III in Individual Support, Certificate IV in Ageing Support or equivalent.


• Assist Clients with Medication (HLTHPS006), preferred.


• Follow Basic Food Safety Practices (HLTFSE001).


• Adaptability, initiating action, collaboration, managing work (including time management), work standards and stress tolerance.


• Demonstrated person-centred care and respectful approach to patients, visitors and staff.


• Previous experience in a similar role would be preferred, but is not essential.
