Senior Health Promotion Practitioner - Grade 3 - Community Health
Are you an experienced Health Promotion Practitioner looking to lead meaningful primary prevention work that improves health and wellbeing outcomes across the Yarra Ranges?
About the role
An exciting opportunity exists for an experienced Health Promotion Practitioner to join Healesville Hospital and Yarra Valley Health in a 12-month parental leave cover position.
This role offers the chance to lead prevention and population health initiatives across the Yarra Ranges within an integrated community health setting. As a senior team member, you will drive the planning, implementation, evaluation and reporting of Eastern Health's commitments under the Integrated Health Promotion Plan, while contributing to broader primary prevention priorities across the region.
Working alongside experienced health promotion, community health and healthcare leaders, you will further develop your leadership, project management and prevention expertise, supporting the integration of prevention into health service delivery and community settings to improve health and wellbeing at scale.
You will collaborate with local government, community organisations, regional partners and internal stakeholders to design and deliver evidence-informed initiatives that address the social determinants of health and improve population health outcomes across diverse communities.
This role is suited to an experienced Grade 2 practitioner ready for the next step, or an established Grade 3 practitioner seeking to contribute to innovative prevention work within a supportive and flexible team environment.
You will bring strong health promotion knowledge and experience, with the ability to apply evidence, policy directions, local data and community priorities to create sustainable change. The role also includes partnership development, staff and student supervision, and contributing to organisational leadership in health promotion and prevention.
About you
To be successful in this role you will possess:
Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
