Senior Clinician Physiotherapist or Occupational Therapist – Community Rapid Triage Team
About the role
As a Senior Clinician within the Community Rapid Triage Team, you will play a pivotal role in supporting clients transitioning from hospital to community-based rehabilitation services. Working autonomously within a highly collaborative and supportive multidisciplinary team, you will conduct timely triage, comprehensive assessments, goal setting, and discharge planning for clients referred to the Community Rehabilitation Program following discharge from Eastern Health hospitals.
In this role, you will identify and respond to clinical deterioration, escalating concerns to medical staff as required to support client safety and help prevent avoidable hospital readmissions. You will also contribute to holistic discharge planning by building strong partnerships with community nursing services, Post-Acute Care, HARP Coordinators, and allied health professionals to ensure seamless continuity of care.

Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role, and an offer is conditional on this being up to date prior to employment.
