Schedule: Monday–Friday + On-Call Rotation

We are expanding our team and seeking a dedicated Licensed Practical Nurse (LPN) to join our home hospice program.

About the Role

The Licensed Practical Nurse performs designated nursing functions under the direction 

of the Registered Nurse. Responsibilities include following the plan of care, providing 

treatments, and working collaboratively with the members of the team to help meet

positive patient care outcomes. 

Key Responsibilities

• Provide in-home nursing care under the direction of the RN Case Manager
• Perform assessments, monitor symptoms, and report changes in patient condition
• Administer medications, treatments, and comfort-focused interventions
• Educate patients and families on care plans, medications, and hospice philosophy
• Document all care accurately and promptly in the EMR
• Participate in on-call rotation as needed
• Collaborate with the interdisciplinary team (RN, social worker, chaplain, aides)
• Support patients and families with empathy, professionalism, and respect

Qualifications

• Active LPN license in the state of Indiana
• Strong assessment and communication skills
• Ability to work independently in the field
• Compassionate approach to patient and family support
• Reliable transportation and willingness to travel within the service area
