Job Description - 1 Full Time Term Post Majority Support Services (PMSS) Worker

Position Summary:


The incumbent is responsible to provide post majority support services in the community of Sandy Bay Ojibway First Nation. The PMSS Worker will report directly to the Post Majority Support Services Supervisor of Sandy Bay Child and Family Services Inc.


 


Some Duties include:



  • Complete assessments to identify participants eligibility for Post Majority Support Services.

  • Develop individualized service plans and SMART goals that promote independence, self-sufficiency, and successful transitions into adulthood.

  • Provide ongoing case management, mentorship, and guidance to support participants in achieving their personal, educational, employment, housing, and wellness goals.

  • Develop and promote a mentorship program.

  • Assist participants in accessing community resources, cultural programming, education and training opportunities, employment supports, housing services, and other relevant programs.

  • Advocate on behalf of participants and collaborate with community agencies, service providers, families, and support networks to ensure coordinated service delivery.

  • Encourage participation in cultural, recreational, and community-based activities that foster personal growth, connection and well-being.

  • Monitor participants progress, maintain accurate case notes and documentation, and regularly review and update service plans and goals.

  • Support participants in developing essential life skills, including budgeting, communication, problem-solving, decision making, time management, and self advocacy.

  • Prepare reports and maintain confidential records in accordance with program policies and procedures.

  • Promote a strength based, culturally safe, and trauma informed approach in all interaction with participants.

  • Be a resource to front-line staff in transitioning young people out of care.

  • Plan and develop programming and workshops for young adults as required.

  • Will document reasons for terminating contract and complete a closing summary.

  • Build and maintain strong working relationships with community agencies, schools, training centers, medical providers, and justice systems.

  • Maintain up-to-date knowledge of child welfare practices and legislation.

  • Conduct outreach and engagement activities with young adults who are eligible for PMSS services.

  • Will follow Sandy Bay Child and Family Services Inc. internal review and approval process.

  • Maintain compliance with Post Majority Support Services Tool Kit.

  • Ensuring administration tasks are up to date and completed as necessary (e.g. brochures, resource manuals, program proposals, internal policies, etc.).

  • Maintain accurate and detailed written case notes and records of each visit, conversation, and any other form of communication (information is not attached to CFSIS) and keep a hard copy file only.

  • Other duties as assigned.



Qualifications:           



  • S.W., B.A., or Child & Family Services Diploma and/or equivalent combination of education and training/experience in a related field of service.

  • MCSW Registration would be considered an asset.

  • Minimum of two-years’ of experience working in a child welfare setting and/or a related field of study.

  • Must have a valid Manitoba Drivers Licence, access to a vehicle and willing to travel. Will provide a current Drivers Abstract. 

  • Must pass Criminal Record Check, Vulnerable Sector Search, and Child Abuse Registry Check.

  • Ability to speak and/or understand the Ojibway language would be an asset.



  • Knowledge and respect of First Nations traditional culture and traditional philosophy.
