Job Description - HCC Nurse

RESPONSIBILITIES



  • Develops, coordinates and implements in-home nursing services under the direction of the HCC Nurse Manager, and in conjunction with KO community Nurse in Charge and community Health Director.

  • Works with a team of Personal Support Workers (PSW’s), unregulated healthcare providers, and other healthcare professionals in the program.

  • Maintains a relationship with the medical professionals in the community, the Chief and Council, and other stakeholders.

  • Maintain current skills through attendance at workshops, in-service and staff meetings.

  • Assists the HCC Nurse Manager with preparing reports for the KO Director of Nursing (DON) and annual year end reports for Health Canada.

  • Performs other duties requested by HCC Nurse Manager, KO Health Director, or DON.

  • Charting using the Tellus. 


KNOWLEDGE, SKILLS, AND ABILITIES



  • A Registered Nurse (RN)/ Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.

  • Minimum 2 years’ experience delivering nursing care in a home or community setting.

  • Advanced Foot Care certified; Mental Health nursing an asset.

  • Knowledge on culture and tradition of remote First Nation communities.

  • Excellent verbal and written communication skills. Ability to speak Ojibway, Oji-Cree, or Cree an asset.

  • Ability to prepare and submit reports.

  • Basic computer skills and willing to learn a new Tellus system.

  • Awareness of working with unregulated Health Workers; Transfer/Delegation of Skills, processes and requirements.

  • Other duties as required

  • Schedule: Rotational basis 2-4 weeks in communities. To be discussed
