Reporting to the Nurse – Manager Iqaluit Home & Community Care, the Home & Community Care Worker II is a cornerstone of Nunavut’s health care system, ensuring that vulnerable clients receive safe, dignified, and culturally appropriate care in their homes. The position provides essential personal care and specialized support, including assistance with bathing, grooming, mobility, medication reminders, and individualized care for palliative, rehabilitation, and post-acute clients. Through these responsibilities the position plays a vital role in sustaining community health and advancing the Government of Nunavut’s commitment to equitable and culturally safe health services.
The HCCW II will be responsible for:
Ensuring that all client, family and program information, records and communication are treated with strict confidentiality.
Carrying out the client care and/or service plans by following the pre-determined task list and assists clients with activities of daily living according to the care plan and task list.
Performing specialized tasks or activities of daily living following individualized, client-specific training, including special care for palliative, rehabilitation and post-acute clients.
Preparing nourishing meals and special diets, plans meals with the client, assists with grocery shopping, performs light housekeeping duties, does light laundry and essential mending of clothing, and assists in activities of daily living such as watering plants and feeding pets.
Recording data appropriately, accurately, completely and in a timely way, observes the client’s condition and reports changes in planned hours of service
The knowledge, skills, and abilities required for this job are usually obtained with a minimum Grade 10, or equivalent along with two (2) years of home care experience. Experience providing personal care for family or community members, including Elders or individuals with health challenges is also required. Basic Cardiac Life Support, First Aid Certificate and Training in Personal Care Skills or home support will be considered as assets including basic computer skills for documentation. This is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required.
The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Written and verbal fluency in Inuktut and English is required. Knowledge of Inuit communities, culture, land, Inuit Qaujimajatuqangit, Inuktut and experience working in a northern cross-cultural environment are also considered assets.
