Department:  Movement Disorders Clinic 

Reports to:  Manager KBI Ambulatory Clinics

Salary Range:  $43.47 to $54.34 

Hours: 37.5 hours per week 

Shifts: Week days 9-5

Status: Temporary full time one year contract (With possibility to convert into permanent full-time)  

Closing Date: June 24, 2026



Position Summary   

The Social Worker in the Movement Disorders Clinic provides clinic-based, time-limited psychosocial support and system navigation for outpatients whose movement disorder creates barriers to accessing, participating in, or sustaining care. Working collaboratively with patients, care partners, physicians, and the interprofessional team, the role focuses on assessment, supportive counselling, education, and coordination with health and community services to address disease-related psychosocial, financial, logistical, and care-planning needs. The position emphasizes consultative intervention, clear role boundaries, and timely connection to appropriate ongoing community and system supports rather than longitudinal case management. 

Duties 

  • Conducting focused psychosocial assessments to identify movement disorder-related barriers affecting access to care, treatment adherence, safety, coping, and participation in clinic follow-up 

  • Providing time-limited supportive counselling, education, and problem-solving for patients and care partners adjusting to diagnosis, disease progression, and changes in functioning 

  • Assisting patients and families with navigation of health, community, financial, and disability support systems, including access to benefits, transportation resources, and community-based services 

  • Supporting early care planning and system readiness for patients with complex needs, including those with atypical parkinsonism, cognitive or functional decline, limited informal supports, or increasing caregiver burden 

  • Providing education to patients and families regarding community care pathways, long-term care processes, and the roles of external care coordination services, while maintaining appropriate boundaries regarding service ownership 

  • Collaborating with the interprofessional team to clarify psychosocial concerns, contribute to care planning, and facilitate communication with relevant internal and external providers and agencies 

  • Facilitating family or team discussions when psychosocial, system, or care-planning issues are affecting the patient’s ability to remain engaged in care 

  • Documenting assessments, interventions, referrals, and recommendations, and arranging brief follow-up when required to complete a defined consultative intervention 

  • Providing clinical teaching to learners and colleagues, and participating in quality improvement, program development, and other relevant clinic initiatives as appropriate 

Qualifications

  • Completion of a Masters Degree in Social Work  
  • Registration with the Ontario College of Social Workers and Social Service Workers  

  • Membership in the Ontario Association of Social Workers, preferred 

  • Three (3) years experience working with an inter-professional team in a hospital setting, preferred 

  • Experience in outpatient or ambulatory care, neurology, rehabilitation, or complex chronic disease populations preferred 

  • Excellent verbal and written communications skills 

  • Excellent interpersonal, time management and organizational skills 

  • Excellent problem solving skills 

  • Research experience/education an asset 

  • Student education/supervision an asset 

  • Knowledge of policies, legislation, program's and issues related to social welfare institutions and services 

  • Knowledge of a second language an asset
