Job Description - Systems Navigator

Department: Health and Wellness


Classification/Level: CS-3


Employment Status:  Regular Full-Time

Reporting Relationship:


The System Navigator works under the direction of the Mental Health & Addictions Clinic Supervisor and Healing Model Manager.


 


Position Summary:


The System Navigator is responsible for assisting community members navigate through the various services that exist both within and around M’Chigeeng First Nation. The purpose of this role is to facilitate access to services for individuals in need by creating a “one stop shop” whereby individuals can access all their information in one place.  This includes but is not limited to assisting with counselling, health services, addiction services, housing applications, education funding, and social assistance status, etc.


 


Essential Functions:


Daily Responsibilities



  • Will provide a range of supports/services that enhance community member access to detoxification and treatment centres; aftercare; counselling; psychiatry services; legal support; education, and social services.

  • Conduct an Intake with community member to ensure they are connected to the most appropriate services that will meet their individual wellness goals;

  • Coordinate aftercare planning with other support services based on community members needs and autonomy;

  • Assist with implementing the Harm Reduction Philosophy and providing ongoing support to the Wellness Recovery Program, working closely with the lead nurse and doctor(s)

  • Willing to learn and input data into our Electronic Medical System

  • Will be a resource for community members seeking to remove barriers related to access to resources via advocacy support with internal and external service providers.

  • Provide supportive counselling to community members with a trauma informed lens.

  • Provide support services within the framework of client centred recovery and rehabilitation.

  • Attend meetings and appointments to support/advocate in the best interests of the community member(s) well-being

  • Ensure professional competence by seeking consultation and effective use of clinical supervision

  • Partner with other service providers to deliver psycho-educational workshops.

  • Provide regular statistical reports as directed by Clinic Manager

  • Participate in regular internal and external meetings.

  • Other duties/assignments as requested.



Qualifications Skills & Requirements


Preferred



  • Post-Secondary Diploma in Health or Social Services discipline

  • Two (2) years’ work-related experience or lived experience of Mental Health & Addictions

  • WHIMIS, Mental Health First Aid, ASIST Training and CPR & First Aid Certifications

  • Knowledge and experience of Workplace Health & Safety procedures

  • Knowledge of Anishnaabek culture, traditions, and practices



Minimum



  • High School Diploma or Equivalency

  • At least one year work related experience or lived experiences.

  • WHIMIS CPR & First Aid Certifications and a willingness to acquire additional training

  • Knowledge of Anishnabek culture, traditions, and practices



Skills and Abilities:



  • Ability to work with minimal supervision and in a team environment, to maintain effective working relationships with the Mental Health and Addictions staff which includes the Peer Support & Outreach

  • Have a working knowledge of the Harm Reduction Program including medically assisted treatments for addictions and pain management

  • Develop strong partnerships within the community and with the Health and Social Services

  • Knowledge of Microsoft Office, PS Suites EMR and general office procedures

  • Maintain professional attitude when interacting with individuals accessing services

  • Ability to work independently and within a team environment

  • A Good Understanding of Addictions, Mental Health, and Serious Mental Illnesses Challenges

  • Ability to ensure confidentiality and work with highly sensitive information

  • Effective interpersonal and communication skills

  • Excellent problem-solving skills

  • Must be a role model both during and after regular work hours

  • Understanding and fluency in Anishinaabemowin or willingness to learn are strong assets

  • Flexible to work odd hours, overtime and/or on weekends as needed

  • Valid Driver’s License and access to a reliable vehicle



Vulnerable Sector Screening (VSS) Rationale:  M’Chigeeng First Nation and its agents are in a position of trust in services to band member children and/or vulnerable adults, so a safe and secure working environment is mandatory.  Successful applicants upon being made  must provide a VSS clear and/or dated of any convictions relating to sexual interference, sexual assault, or related charges.  The VSS must be current 30-days and must be an original.
