Position Summary
Reporting to the Seniors Services Manager, the Assessor will be working for the HCP@clinic - a community-based health initiative designed to improve access to healthcare for seniors facing barriers to care. Running from November 24, 2025 (date subject to ethics approval) through June 25, 2026, the clinic will operate weekly at 252 Sackville Street in Regent Park.
Each week, seniors in the building will attend the HCP@clinic to complete a Health Risk Assessment or drop in for follow-up care and support provided by a Research Nurse, an Assessor and two Peer Workers. The primary goal of this initiative is to help seniors remain healthy and independent by strengthening their connection to primary care, community resources and health education.  

GENERAL ACCOUNTABILITIES
The Assessor will play a key role in supporting senior clients of the HCP@clinic. Working under the guidance of the Research Nurse, the assessor will conduct health risk assessments with clients to identify areas of need. The assessor will then share findings and recommendations based on assessment results with peer workers. The health risk assessment has been developed by the HCP@clinic research team. This team will be providing training on the assessment tool.

Duties and Responsibilities
  • Meet clients 1:1 to conduct a health risk assessment
  • Provide the client recommendations based on assessment results
  • Support clients in understanding and following care recommendations
  • Provide health promotion and educational materials
  • Provide summary of client interactions and recommendations to peer workers to facilitate system navigation
  • Document client interactions in the database every week
  • Report to supervisor about client interactions and observations
  • Other duties as required.
Qualifications
  • Familiarity with Downtown East Toronto and the resources available will be considered as an asset
  • Good understanding of the systemic and health access barriers in Toronto, for older adults
  • Ability to communicate complex information in a clear and compassionate manner
  • Ability to listen to clients and help them become comfortable to share their challenges
  • Willingness to follow all Dixon Hall workplace policies and procedures
  • Comfortable working both independently and as part of a team
  • Can follow through on commitments
  • Strong verbal and written English language skills; fluency in other languages is an asset especially Cantonese, Mandarin or Vietnamese
  • Comfortable using technology for communication, coordination and data entry
  • Background in health-related assessments is an asset
  • Must be qualified to work in Canada legally for the employer.
  • Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.
