About the role


 


The role of the Prostate Cancer Specialist Nurse (PCSN) at Goulburn Valley Health (GVH) will be to work in partnership with both internal and external health care providers in order to optimally manage the needs of prostate cancer patients within the GVH catchment. This will include care in the both the public and private sectors and across the entire disease trajectory. Navigating the healthcare system can be complex and a daunting task for patients and families. 


 


The PCSN will become a point of contact and key advocate by providing coordinated, evidence based, interdisciplinary, seamless and patient focused nursing care and the role will have a strong focus on supportive care assessment and referral. The PCSN will be a primary resource for both internal and external providers and will deliver education and instigate quality improvement activities. The incumbent will work in partnership to explore, develop and implement evidence based and patient focused model of care and pathways that are relevant and appropriate improvements on current practice.
