  • Support and implement HSE and GMS initiatives, including preventative care and chronic disease pathways.

  • Develop and manage patient-centred treatment plans for acute and chronic conditions.

  • Work collaboratively with nursing staff, allied health professionals, administrative teams and external healthcare providers.

  • Demonstrated experience managing GMS patient populations.

  • Experience delivering or leading HSE Chronic Disease Management (CDM) Programmes.

  • Strong understanding of HSE primary care initiatives, GMS contracts and chronic disease pathways.

  • Excellent clinical, communication and organisational skills.

  • Ability to work independently and collaboratively within multidisciplinary teams.

  • Full clean driving licence and access to own transport.

Desirable Experience

  • Experience in nursing home medicine, elderly care and complex chronic disease management.

  • Experience leading service development and quality improvement initiatives.

  • Interest in population health management and integrated community care.

What We Offer

  • Competitive salary of €114,200 – €135,000 per annum.

  • Full Medical Indemnity Insurance cover.

  • Annual Irish Medical Council registration fees paid by the organisation.

  • Ongoing support for the delivery and development of the HSE Chronic Disease Management (CDM) Programme.
