GENERAL DESCRIPTION
Responsible for rendering professional nursing care to patients within an assigned unit of the hospital. Such nursing care is an integral part of the individualized holistic patient care plan as directed by the medical staff according to the Policies and Procedures of the hospital and in keeping with the generally accepted principles, policies and procedures of the Nursing Services.
JOB DESCRIPTION
1.Adherence to nursing care vision, mission, goals and objectives, aligns with the scope of nursing services, and performs shift duties as assigned by the Unit Manager to ensure high quality care rendered.
2.Notifies the unit manager of any changes in the situation that could impact the standards of care.
3.Obtains and handovers a report of patient progress, ensuring all doctor's orders are followed before changing or over taking shifts and notifying any incidents or problems encountered.
4.Visit patients in the unit with team members to assess conditions, re-plan nursing care, and determine nursing priorities based on patient needs and staff availability.
5.Provides comprehensive hemodialysis patient care using the nursing process approach and various medical devices (hemodialysis machines, CRRT, various access sites), manages all phases of hemodialysis treatment, maintains patient records, and promptly reports abnormalities to medical professionals. 6.Conducts nursing rounds on patients, providing emotional support and accompanying doctors while doing rounds, as well as assisting in procedures as needed.
7.Executing the doctor's orders, coordinating with team members related to patients’ treatment, and fostering effective communication with all disciplines, patients, and relatives to ensure patient satisfaction.
8.Engages actively in health education for patients and their families, unit meetings, and hospital activities, providing support and education to dialysis patients, advising on organ donation programs, and planning treatment programs to ensure adequate treatment.
9.Provide clear, compassionate, and culturally sensitive communication with patients, ensuring their understanding of their condition, treatment options, and care plan in a comprehensive manner.
10.Assist with patient billing procedures, scheduling next appointments and guide them to the cashier for payment processing after receiving treatment. 11.Collaborate with the unit manager in administration, setting care standards, and fulfilling duties in the absence of the unit manager, while also carrying out assigned tasks and providing feedback.
12.Charge accurately, incorporating charges for equipment and consumable items into the patient's charges and verifying their completeness.
13.Inspect and document the supply of sterile packs to ensure the integrity of the packaging and maintain an adequate supply of unit stock from CSSS.
14.Participates in the review and revision of nursing policies, procedures, and work instructions, providing valuable insights, promoting best practices for optimal patient care, and continuously improving the process.
14.Operate with proficiency in implementing hemodialysis unit work instructions, nursing policies, and procedures to ensure optimal care for patients undergoing treatment.
15.Inspect the DDA drugs with the previous shift's nurse in charge and adhere to hospital policies and procedures in medication care, custody, and administration.
16.Checks daily and audits resuscitation trolley periodically as scheduled. 17.Promotes infection prevention and control by ensuring staff follow protocols, implementing surveillance activities, demonstrating commitment through personal involvement, and monitoring patients using appropriate care bundles. 18.Maintain confidentiality of patient’s personal and medical history.
19.Operates and maintains specialized equipment in a unit, guiding new staff on the use of it and maintaining accurate records.
20.Conducts comprehensive pre-use checks on vital equipment, such as ventilators, to ensure their safe and optimal functioning and readiness for use. Collaborate with the unit manager in maintaining equipment, medical supplies, and drugs, ensuring proper functioning and service according to the PPM schedule.
21.Notify biomedical personnel of faulty equipment and assist in maintaining instruments and equipment through proper cleaning, handling, disinfection, and usage.
22.Participate in a quarterly stock count with the finance staff.
