PACE Enrollment Specialist

Talent Connect Executive Search & HR Solutions is recruiting for a PACE Enrollment Specialist to support a growing healthcare organization serving older adults in the San Fernando Valley.

This is a community-facing role ideal for someone who understands the San Fernando Valley, enjoys building relationships, and has experience in healthcare outreach, enrollment, community engagement, marketing, or field sales.

The Enrollment Specialist will help educate seniors and their families about the Program of All-Inclusive Care for the Elderly (PACE), develop community referral relationships, generate prospective participant leads, and guide eligible individuals and their families through the enrollment process.

Key Responsibilities

• Conduct ongoing community outreach throughout the San Fernando Valley to increase awareness of PACE services.


• Build and maintain relationships with community organizations, senior resources, healthcare providers, referral partners, and other local organizations.


• Identify new referral sources and generate qualified prospective participant leads.


• Educate seniors, families, caregivers, and community partners about PACE services and eligibility.


• Follow up promptly with prospective participants and families.


• Maintain an active pipeline of prospective participants and work toward established enrollment and outreach goals.


• Coordinate prospective participant visits, assessments, and enrollment activities.


• Work closely with the PACE interdisciplinary team and operational staff throughout the enrollment process.


• Maintain accurate documentation of leads, outreach activities, referral sources, and prospective participants in the CRM.


• Participate in community presentations, educational events, and outreach activities, including occasional activities outside normal business hours.


• Ensure marketing and enrollment activities comply with applicable PACE, state, and federal requirements.

Qualifications

• Bilingual English/Spanish required.


• Must live in or near the San Fernando Valley and have strong familiarity with the local community.


• Bachelor’s degree in Healthcare, Sociology, Business Administration, Marketing, or a related field preferred; equivalent relevant experience will be considered.


• Minimum 2 years of experience in field sales, healthcare marketing, community outreach, public relations, enrollment, or a related area preferred.


• Strong community networking and relationship-building skills.


• Demonstrated ability to generate leads, conduct outreach, and successfully follow prospects through a process.


• Strong verbal, written, presentation, and interpersonal communication skills.


• Comfortable working directly with seniors, families, caregivers, healthcare professionals, and community organizations.


• Knowledge of senior healthcare, managed care, PACE, Medicare/Medi-Cal, or community-based senior services is highly desirable.


• Ability to work independently and spend time in the field conducting outreach and prospecting.


• Valid California driver’s license and ability to travel locally as required.
