Key Responsibilities

• Conduct assessments to identify the needs and strengths of clients.
• Develop and implement tailored support plans in collaboration with clients.
• Provide guidance and resources to help clients access necessary services.
• Monitor clients' progress and adjust support plans as needed.
• Advocate for clients within the community and collaborate with other service providers.
• Maintain accurate documentation and records of client interactions and progress.

Requirements

• A degree in social work, psychology, or a related field.
• Proven experience in a similar role or working with vulnerable populations.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Empathy and a non-judgmental attitude towards clients.

Nice to have

• Knowledge of local support services and resources.
• Experience with case management software.
