This is a travel, temporary assignment in a long-term care setting with a shared rotation across two sites, providing rehabilitation services to adult residents.

Responsibilities
- Conduct comprehensive OT assessments for residents to identify functional limitations and rehabilitation goals.
- Develop and implement individualized treatment plans focused on activities of daily living, mobility, upper-extremity function, and cognitive/psychosocial goals as appropriate.
- Deliver direct therapy to residents, monitor progress, adjust treatment plans, and document outcomes in the medical record.
- Collaborate with an interprofessional team (nursing, PT, speech, social work) to coordinate care, participate in care planning, and support discharge planning when applicable.
- Educate residents and families on home exercise programs, safety strategies, and equipment use; ensure proper documentation for reimbursement and regulatory compliance.

Requirements
- Current Occupational Therapist license in Washington state (or eligibility for licensure to practice in WA)
- Degree in Occupational Therapy from an accredited program (OTR not required here, but a licensed OT is expected)
- Ability to work in a shared assignment across two long-term care sites, with a 3-day/2-day rotation
- Strong clinical assessment, documentation, and communication skills
- Comfort delivering rehabilitation services in a geriatric, long-term care setting
