Responsibilities
- Conduct PT evaluations and develop individualized treatment plans for residents
- Implement therapeutic interventions including gait training, balance, mobility, strength, and functional activities
- Monitor progress, document outcomes, and adjust plans of care in the electronic health record per policy
- Collaborate with an interprofessional team (nursing, therapy colleagues, and ancillary staff) to optimize resident outcomes
- Educate residents and families on exercises, safety, and home programs to support continued improvement
- Maintain patient safety, adhere to infection control practices, and ensure compliance with standardized protocols

Requirements
- Active Physical Therapist license in the United States; NH licensure or eligibility to obtain NH licensure
- Graduation from an accredited physical therapy program
- Ability to perform duties in a skilled nursing facility setting and interact with geriatric populations
- Flexibility to work a 5x8 day shift and participate in a 13-week travel assignment
- Strong communication, documentation, and collaboration skills
