Overview
Social Worker – Case Management (MSW/RN) – Everett, WA. Travel assignment in a Level II trauma center with two nearby campuses. Start date 05/26/2026 for an 8-week assignment. Shift: 4x10 days (day shift). Weekly pay range: $1,566–$1,602 per week. Guaranteed hours: 72 hours per 2-week pay period. This role supports adult and geriatric patients with discharge planning and care coordination within Med-Surg and ED settings, with EPIC charting and close interdisciplinary collaboration.

Responsibilities
- Conduct psychosocial assessments and develop discharge plans for adult and geriatric patients in Med-Surg and ED
- Coordinate care with the interdisciplinary team (PT, Interpretation Services, Social Services, Pharmacy, Transportation)
- Document assessments and care plans in EPIC; ensure timely updates and accurate charting
- Facilitate patient and family education on resources, supports, and post-discharge needs
- Manage transitions and referrals to outpatient or community services to support continuity of care

Requirements
- Education and licensure: MSW with WA LMSW/LCSW eligibility or WA RN license; BLS for RN track
- Licensure: WA LMSW/LCSW or WA RN license as applicable
- Experience: Minimum 1 year of acute care experience with discharge planning/care coordination; strong preference for case management across Med-Surg, long LOS, and ED
- Certifications/skills: EPIC charting experience; ability to float between hospital areas and to a sister campus; comfort with adult/geriatric patient populations
- Scheduling and holidays: Weekend obligation (every other weekend); holiday coverage required (3 of the 5 major holidays: Thanksgiving, Christmas Eve, Christmas Day, NYE, NYD); active WA licensure and eligibility for Nursys as applicable
- References: Minimum 2 supervisory references covering 1 year of employment within the last 3 years
