MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. High School Diploma or GED equivalent

1. Develops relationships with participating physicians and service providers.

2. Liaising with medical providers.

3. Guiding patients to appointments

4. Coordinating the logistics for plan adherence-appointments

5. Supporting patient family members
