• Plans
  and implements programs on all units throughout the facility daily. These programs support, maintain, or improve
  the emotional, spiritual, and/or social behavior of each resident to promote
  growth and development of each individual resident. Groups are resident focused and include; recreational,
  religious, intellectual, social, service orientated and exercise type
  programming.

• Develops
  monthly organizational plans for assigned programs including goals, needed
  supplies and methods of presentation.

• Provide
  one to one therapeutic programs to individuals that are at times unable to
  participate in structured programs; i.e.; poor attention span, restlessness or behaviors.

• Transports
  residents to and from programs, pushing wheelchairs and activity carts.

• Provides
  input to the Activity Director regarding residents progress during programming,
  assisting the Activity Director by completing paperwork related to Care
  planning needs; Activity Attendance and Activity Progress notes.

• Documents
  progress and responses of residents during programming in order to fulfill
  state requirements.

• Works
  with community groups and volunteers, accompanying residents on outings in the
  Sunny Ridge Van. Valid Drivers License
  to drive the facility van.

  Operates all
  equipment in department including music system, DVD players, sewing machines and maintains
  inventory on supplies.

• Responsible
  for physical set-up, clean-up and safety in the department sorting, packing and
  moving supplies.

• Assists
  in the orientation of new Sunny Ridge Health and Rehabilitation Center volunteers
  by explaining the principles and activities of the activity department.

• Assist
  in identifying potential at-risk situations; i.e. poor safety awareness, falls,
  personal harm, harm to others, restlessness, wandering.

• Performs
  other duties as assigned.
  
  

• Ability
  to verbally communicate with the residents and residents families, and to
  exercise tact in dealing with problems concerning residents care.

• Performs
  repetitive motions including bending, twisting, stooping, pushing and pulling.

• Ability
  to stand for an extended period of time without a significant rest period.

• High
  school diploma or general education degree (GED); or one to three months
  related experience and/or training; or equivalent combination of education and
  experience.

• Ability
  to work flexible hours and variable shifts, every other weekend and any other
  expectations based on resident needs.

• Some
  knowledge of modern methods and practices of activity therapy as applied to
  residents in a skilled care institution.

• Some
  knowledge of activity therapy materials and equipment.

• Some
  knowledge of Sunny Ridge Health and Rehabilitation Center routines and
  procedures.

• Is
  creative and flexible. Is able to work
  with a variety of personalities and provide a variety of activities
  simultaneously yet able to provide structure and routine. Is able to create a calming environment. Is able to improvise and reformat daily
  activities based on resident needs, moods and behaviors.

• Ability
  to secure the cooperation of residents in activity health situations.

• Ability
  to make simple analysis regarding the program and response of residents.

• Ability
  to push wheelchairs, and transport residents in and out of facility (bus/van)
  for activity needs.

• Ability
  to read, write and comprehend simple instructions, short correspondence, and
  memos.

• Ability
  to effectively present information in one-on-one, to customers, clients, and
  other employees of the organization.

• Some
  knowledge of PC operations and software are required.

• Some
  typing skills are also required.
