Salary - $95k-110k

Shift - 9:00am to 5:00pm, Monday thru Friday, MOD and on-call rotation

The Clare is recruiting for a hospitality focused Admissions & Outreach Manager to join our team! This position is responsible for leading and managing the entire admissions process for The Clare’s Skilled Nursing, ensuring smooth transitions for new residents and maintaining census targets. This position plays a pivotal role in assessing, approving, and coordinating all admissions, serving as the primary liaison between referral sources, families, and internal departments. 

Here are a few of the daily responsibilities of an Admissions Nurse Manager:

• Oversee and manage all aspects of the admissions process, from initial inquiry through move-in. 


• Evaluate and approve potential residents based on clinical, financial, and operational criteria. 


• Complete onsite or virtual pre-admission assessments at hospitals and other care settings.


• Collaborate with the LCS Regional Health Care Marketing and Sales Director to ensure consistency in LCS standards and best practices and adhere to SalesFIRST Standards. 


• Serve as the primary decision-maker on patient and resident admissions. 


• Develop and maintain strong relationships with hospitals, physicians, discharge planners, and community partners to ensure a steady flow of qualified referrals. 

Here are a few of the qualifications we need you to have:

• Bachelor's degree in Nursing, or related field (Master’s preferred)


• Minimum of 3–5 years of experience in admissions, intake, or case management in a senior living, long-term care, or healthcare setting.


• Experience in evaluating clinical information and making placement decisions.


• Strong knowledge of Medicare, Medicaid, and insurance requirements related to admissions.


• Proficiency with CRM and EMR systems; familiarity with PointClickCare a plus.


• Proficiency with Microsoft Office Suite products
