The Adult Care Manager is responsible for coordinating comprehensive, person-centered care for adults who have complex medical, behavioral health, and psychosocial needs. This role provides community-based care management services, including assessment, care planning, service coordination, and ongoing monitoring to improve health outcomes and quality of
life.The Care Manager collaborates with healthcare providers, behavioral health clinicians, social service agencies, and community partners to ensure integrated care across systems. They support individuals with high-risk or complex needs, connect them to resources, and help reduce barriers to care while improving overall health outcomes.
Working Conditions: This position will be located at the Manhattan office at 33 West 60th Street, New York, NY 10023, Suite 600, or any of our other New York State clinics. This is a professional work environment where a computer, desk, chair, and office supplies will be provided onsite at each of our offices.

While performing the duties of this job, the employee is regularly required to hear and speak on the phone or computer throughout the day, stand and sit for an extended period of time and be able to write and answer emails in addition to carrying objects no greater than 25 pounds, when needed.
