New Friends New Life restores and empowers trafficked and sexually exploited teen girls, women, and their children, and drives awareness of the issue and its prevalence. By providing access to education, job training, interim financial assistance, mental health, and spiritual support, New Friends New Life helps women and children overcome backgrounds of abuse, addiction, poverty, and limited opportunities.
Position Overview
The Adult Case Manager is responsible for a caseload of up to 20 to 25 women in our Adult Resource Center (ARC). The key responsibilities for this role include guiding safety planning; needs assessments; connecting clients to resources; assisting with groups and events and on-going support; and working collaboratively with other direct service team members.
Work Schedule
Monday-Thursday, 9am-5pm (two shifts of noon-8pm per week will be assigned to cover evening needs)
Fridays, 9am-3pm
Job Responsibilities
Create and implement a plan of action for women who have been sex trafficked and/or commercially sexually exploited in order to assist and support them through their restoration process.
Implement trauma informed case management in connecting members to community resources such as housing stability; safety planning; medical access; crime victim's compensation; education; and employment.
Determine clients’ requirements via intake interviews, and assess, develop, monitor, and evaluate member care plans and progress.
Maintain client records by reviewing case notes; and logging events and progress.
Monitor cases by verifying attendance; advocating for needed services; obtaining additional resources; intervening in crises; and providing personal support.
Communicate client progress by conducting and participating in weekly interdisciplinary meetings and evaluations, disseminating results and working through obstacles with the therapeutic team.
Work with clients on an individual and group basis to help them develop life management skills, and assist the team with evening programs and event activities.
Ensure safety and compliance with all agency and client-related protocols, policies and procedures.
Perform other duties as requested.
Requirements:
Bachelor’s degree required; degree in Social Work preferred
Minimum 1 year of related case management experience
Proven experience working with high-risk youth populations and a working understanding of trauma-informed care
Titanium recordkeeping experience a plus.
Ability to maintain confidentiality of those on their caseload
Ability to manage multiple projects simultaneously with high attention to detail
