❤️ What You'll Be Doing

As a Case Manager, you'll be more than a service provider; you'll be a coach, advocate, problem solver, and trusted support person.

You'll:

✅ Help individuals identify goals and create plans for success
✅ Connect people with community resources and support services
✅ Advocate for clients and help remove barriers to independence
✅ Partner with providers, families, and support networks
✅ Support skill building, recovery, and personal growth
✅ Maintain quality documentation and service records
✅ Celebrate victories, big and small, with the people you support

You're a Great Fit If You:

✔ Have a High School Diploma or equivalent
✔ Hold an MHRT-C or MHRT-C Provisional Certification (or are eligible to obtain MHRT-C)
✔ Have experience in behavioral health, mental health, social services, or human services
✔ Have at least one year of experience supporting individuals with mental illness and/or developmental disabilities
✔ Enjoy building relationships and solving problems
✔ Can work independently while also being a great teammate
✔ Have a valid driver's license and reliable transportation
