Adult Day Center Nurse

Alzheimer’s Community Care (“ACC”) is a non-profit organization that provides support and services to families, caregivers, and patients of Alzheimer’s disease and other conditions resulting in dementia.

Responsibilities:

• Provide coordination and supervision of basic health services and needs of patients in the Specialized Alzheimer’s Day Center


• Administer patient medication as directed by prescription label, physician orders, and accepted nursing practices


• Evaluate and supervise each patient’s health needs and assist in the design and implementation of the activity schedule


• Develop individual care plans and identify recurrent patient problems, symptoms, and behavior changes in written progress notes


• Alter care plan to meet changing patient and family needs


• Interact with patient physicians when needed


• Assist with and supervise help given for patient’s personal care needs including toileting, dressing, eating, and ambulating


• In the absence of the Program Manager, act as the Program Manager and supervise program staff and volunteers

Requirements:

• Clear, active Florida LPN license


• 2-5 years’ experience in a clinical setting (or psychiatric/crisis intervention background) preferred, although recent graduates will be considered


• Must pass AHCA level II criminal background and drug screenings

• Ability to handle multiple tasks simultaneously and still maintain flexibility


• Excellent time management skills


• Proficient in all Microsoft Office suite products


• Work independently and as part of a team
