The Ambulatory Clinical Quality and Accountable Care Organization (ACO) Manager will be a registered nurse or other clinical personnel with in-depth knowledge and background of nursing theory, expert skills in nursing practice, stellar communication abilities, in-depth understanding, and autonomy to develop and manage operational initiatives with measurable outcomes and operations programmatic activities for overall quality and the ACO participation.  This manager will collaborate with Hamilton Physician Group and other affiliates (Hamilton Physician Services [HPS]) Administration and Practices to be responsible for the development and management of quality project activities including, but not limited to; (a) comprehensive wellness programs (b) quality measure monitoring and improvement (c) clinical improvement programs (d) overall clinical quality, and (e) other programs relative to quality patient care. The position is responsible for ongoing training of clinical quality and ACO related information to clinical providers and is a liaison between the ACO and clinical staff to improve workflow efficiencies and quality measures.  This manager will collaborate with HPG/HPS Practice Managers, Directors, the Executive Director, and the Vice President to facilitate quality metrics, ACO, and other third-party payor value-based reporting. The Manager is responsible for creating protocols and policies/procedures.  The manager works in a matrix relationship providing supervision for all departmental specific clinical support staff including Care Coordinators, Senior Care Coordinators, Social Workers and other associates in ambulatory clinic settings.
