$76,398.00 - $99,320.00The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
GENERAL PURPOSE:FT (40 hours) position available: Monday – Friday days with some late shifts and occasional Saturdays if unit requires.
Seeking a strong candidate focused on promoting health equity and inclusion, committed to supporting children and their families. Will work within an established, 4-person nurse case manager/nurse navigator team to support our providers and interdisciplinary team members while meeting the various needs of children and their families. will play a valuable role in supporting to some of our communities most medically complex children working with these children and their care givers to identify and support mutually agreed upon patient/caregiver/provider developed goals while managing their clinical condition at home to promote improved health care outcomes and quality of life. BSN preferred, 3 years pediatric nursing experience with strong organizational and communication skills as well as experience working within the electronic medical record.
