Summary of Essential Functions

  • The Post-Acute Case Management assistant will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt positive health behaviors. The PACCMA will support providers and case management, social work, and transitions of care staff through an integrated approach to care management and community outreach. PACCMA will promote and improve the health of patients and their families. Provide social support with resource information, resource navigation, provide information about chronic disease prevention program services.

 

Educational Requirements /Certifications

  • High School Diploma or equivalent.
  • Must be able to communicate effectively in English, both verbally and in writing.

 

Qualifications/Knowledge/Skills/Abilities:

  • Experience in community work, education, healthcare, or related field preferred.
  • Ability to follow through on projects for timeliness, completeness, and serviceability.
  • Ability to communicate effectively and courteously with medical and technical personnel.
  • Demonstrate patience, tact, diplomacy, and persistence in daily contact with patients, physicians and their staff, visitors, and all hospital personnel.
  • Ability to demonstrate proficiency in use and application of all electronic tools.
  • Understand and comprehend basic medical terminology.
  • Must be able to prioritize work according to written or verbal instructions using organizational skills and sound judgment.
  • Ability to maintain confidentiality.
  • Ability to handle difficult situations involving patients, families, physicians, regulatory personal in professional manner.

 

Physical Requirements

  • Requires prolonged sitting at workstation, moderate walking.
  • Requires ability to lift various weights frequently; perform moderately difficult manipulative and hand-eye coordination skills; see objects closely; and hear normal sounds with some background noises.
  • Working under stressful conditions and working irregular hours.
  • Requires up to 12-hour shifts and included weekends and holidays.

 

Duties and Responsibilities

  • Understands systems analysis and design, operational processes, data management techniques, reporting tools and methods. Demonstrates ability to see a project or assignment to completion, accepting responsibility for all aspects of the project. Consistently attends meetings.
  • Provides health information in various settings through community outreach, group health, education sessions, individual encounters, telehealth to determine patients and their family’s needs and concerns related to community and their health.
  • Provides referrals to appropriate hospital and community agencies and follows up with individuals to ensure they received the needed services, act as an advocate and liaison between the patient /family and community service agencies including but not limited to Department of Health and Human Services, Health Department, support groups, community navigate resources.
  • Assist patients to access and utilize health improvement resources. Assist patients and families to identify, set, and maintain goals, and motivate patients to be active and engaged participant in their health.
  • Works collaboratively with case managers / social workers/ nurse navigators to identify, assist, and create specific health improvement plans for at risk populations. Assists in delivery of required documents to patients or patient designee. Completes PCS, Transportation authorizations, as directed. Assists with completion of EDO or CMEs for mental health court. Provides informational packets to identified patients and families.
  • Knowledge of community resources and able to effectively explain how to access and navigate identified services.
  • Schedules follow up appointments for patients as determined by compiled reports. Documents according to process for electronic medical record.
  • Uses interpersonal skills which convey a positive and supportive attitude and is straightforward and tactful in language (e.g.: active listening, good communication, telephone etiquette). Demonstrates flexibility in response to unexpected changes in work volume, emergency staffing, and scheduling.
  • Performs all other tasks/responsibilities as necessary.
