Job Description - Behavioral Health Case Manager

Pay: $25 – $34/hour

Connect clients to housing, benefits, and treatment resources within trauma-informed community programs across Sacramento and Placer Counties, including CalAIM-funded services.

What we're looking for:

BA in social work, psychology, or related field (or equivalent experience)
Case management experience with vulnerable populations
Reliable transportation for field work
