Conduct patient intake assessments and assist in developing individualized treatment plans
Provide follow-up sessions and care coordination for patients enrolled in behavioral health programs
Document clinical notes using standardized templates and maintain accurate patient records
Collaborate with psychiatrists and primary care physicians regarding care plans and patient progress
Act as a liaison between the behavioral health care team and primary care providers
Manage patient registry and track time spent on each patient for billing and reporting
Coordinate with patients’ family members when necessary to gather medical and behavioral history
Support care delivery across both clinic locations
Master’s degree in Clinical Psychology, Counseling, or a related field
Master of Social Work (MSW) is also considered
Minimum 2 years of relevant Behavioral health experience
Bilingual in English and Spanish required
Strong documentation, coordination, and communication skills
Ability to work collaboratively with multidisciplinary healthcare teams
