AmeriCARE Denver is currently seeking a dedicated Care Coordinator to join our team at our Greenwood Village, CO location.

• Coordinate and provide care services for clients


• Develop care plans based on individual needs


• Communicate effectively with clients, families, and healthcare providers

• Previous experience in care coordination or a related field preferred


• Strong communication and organizational skills


• Knowledge of healthcare regulations and practices
