Neuron Health Systems is a community-based provider of acute outpatient rehabilitation and medical and non-medical direct care. We deliver individualized, family-centered support — partnering with families and caregivers and applying an interdisciplinary approach so every patient can reach their fullest potential.

The Role

We're looking for a Care Coordinator to be the connective thread between patients, families, clinicians, and physicians. This is a hybrid role — most coordination and documentation work is done remotely, with occasional in-person visits or meetings as needed.

What You'll Do

• Coordinate scheduling of clinical and support services across the care team.


• Serve as the primary point of contact for patients and families regarding services.


• Track authorizations, re-certifications, and service frequency requirements.


• Communicate with physicians and clinicians regarding changes to the plan of care.


• Monitor case status and flag gaps in service or documentation.


• Maintain accurate records in company systems, including HHAeXchange and Salesforce.


• Support case conferences and interdisciplinary team communication.


• Participate in firm accreditation activities and surveys.
  
  
  

What You Bring

• Experience in home care, healthcare coordination, or case management preferred.


• Strong organizational skills and comfort managing multiple cases at once.


• Clear, professional communication with patients, families, and clinicians.


• Proficiency with Google Workspace and care management systems.


• Ability to work independently in a hybrid, remote-first environment.
