SUMMARY
Assists individuals in maintaining an interactive process of informed decision-making about Long-Term Services and Supports. Serves a key role in coordinating the efforts of formal and informal caregivers on behalf of clients. Care Management is a person-centered service that values the consumer’s choices and rights.
