Advanced Houston Specialty Hospital is looking for Care Manager (RN) in our newly renovated facility at 205 Hollow Tree Lane, Spring, TX.


Position Summary: 


Reports to the director of nursing. Directly supervises the clinical coordinator and infection control nurse. Coordinates patient-centered care and works with physicians, nursing staff, interdisciplinary team members, patients, and community providers to address clinical, psychosocial, and discharge needs. Supports continuity of care, patient safety, efficient use of resources, timely care transitions, and compliance with federal and state regulations and Advanced Specialty Hospitals (ASH) policies. 


Essential Functions: 


1. Demonstrate thorough knowledge of hospital and departmental policies and procedures and organizational structure. 


2. Perform comprehensive assessments of patients’ clinical, functional, psychosocial, and discharge needs. 


3. Participates in the clinical on-call rotation and provides consultation, support, and operational coverage during evenings, weekends, holidays, and emergency situations as assigned. 


4. Coordinate services across departments and levels of care to promote continuity and prevent unnecessary delays in treatment. 


5. Identify barriers to care and facilitate appropriate interventions and resources. 


6. Monitor patient progress and modify the care plan as patient needs change. 


7. Participate in interdisciplinary care conferences and treatment planning 


8. Transitions and Discharge Planning 


9. Begin transition and discharge planning early in the patient’s course of care. 


10. Assess the patient’s anticipated needs following discharge 


11. Facilitate communication between the current healthcare team and the providers responsible for the patient’s continued care. 


12. Identify patients at increased risk for readmission and implement appropriate interventions. 


13. Review clinical information, provider recommendations, diagnostic results, medications, and treatment plans to identify potential gaps or barriers to care. 


14. Monitor patients with complex or high-risk healthcare needs. 


15. Promote appropriate and efficient utilization of healthcare services and organizational resources. 


16. Identify avoidable delays in care, treatment, consultation, diagnostic testing, transfer, or discharge. 


17. Collaborate with providers and leadership to resolve barriers that may unnecessarily extend the patient’s episode of care. 


18. Assist with utilization review, authorization, medical-necessity documentation, or payer communication when assigned. 


19. Support organizational initiatives designed to improve quality, efficiency, patient outcomes, and cost-effective care. 


20. Provides direct patient care and may assume assigned clinical staffing responsibilities as needed to support patient care operations. 


21. Demonstrate clinical competence when providing care, using technology, administering medications, performing procedures and managing emergencies, acting as a patient advocate. 


22. Maintain accurate documentation and reporting in accordance with facility policies and applicable laws. 


23. Promote a culture of safety, accountability, and professionalism among staff. 


24. Maintain accountability for staff, patients, and hospital property; adhere to all security and safety protocols applicable to a correctional healthcare environment. 


It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management. 


Education and Experience Requirements: 


Graduate from an accredited nursing program required. Current unrestricted Registered Nurse (RN) license in the state of Texas or compact state required. Basic Life Support (BLS) certification required. Two years’ experience in care management preferred. Experience in correctional healthcare also preferred. Valid driver’s license with an acceptable driving record required, unless waived by management. 


Post Hire Requirements:

Must successfully complete annual requirements to include maintaining licenses, certifications, and passing an annual background check.
