Role Summary
The Care Manager plays a key leadership role in delivering high-quality, client-centered care. This position is responsible for coordinating, implementing, and supervising care services while supporting caregivers and maintaining strong relationships with clients, families, and referral partners.

· Serve in an on-call rotation to support clients and caregivers during evenings and weekends
· Respond to urgent situations, including staffing gaps or client care issues
· Perform caregiver and/or client support, including in-person visits, introductions, and new client assessments
