SUMMARY DESCRIPTION OF WORK


Serving clients across rural communities in Southwest Iowa, the Care Team Specialist plays a key role in improving access to care in areas where services are limited and relationships matter. The position works closely with Zion staff and Heartland Family Service to strengthen coordination, improve data sharing, and support continuity of care. This role operates within a flexible work environment that values collaboration, adaptability, and responsiveness to client and community needs while strengthening the rural mental health care continuum.


The Care Team Specialist serves as a central coordination and support role, ensuring clear communication, timely documentation, and effective collaboration between Heartland Family Service and Zion.   This position supports high quality, client centered care by tracking required assessments, coordinating services, and managing data and reporting requirements tied to the Certified Community Behavioral Health Clinic (CCBHC) model.


Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience).


Work Schedule: 37.5 hours per week (Monday through Thursday, 8:30 a.m. to 5:00 p.m.; Fridays 8:30 a.m. to 2:30 p.m.)

MINIMUM QUALIFICATIONS



  • Bachelor’s degree in psychology, social services, sociology or other related fields desired or comparable work experience preferred

  • One year experience in health and human services, or related field


Essential Duties and Responsibilities



  • May assist with completing releases of information, consents for treatment, and all required agency documentation.

  • Track required assessments, including due dates, completion status, and follow up needs to ensure compliance with CCBHC and agency requirements. These will include PHQ-9, DLA-20, and Columbia Suicide Severity Rating Scale in addition to other assessments as applicable.

  • Insure accurate data collection, entry, and record maintenance

  • Monitor timeliness and accuracy of assessment completion and proactively address barriers to completion with the care team.

  • May assist with gathering the following information and document assessments at admission and ongoing intervals, identifying client strengths, abilities, needs, and goals.

  • Advocate for client needs and promote consistent engagement in services.

  • Connect clients to appropriate resources including physical health, behavioral health and substance use treatment, housing, education, employment, and social services.

  • Coordinate with the client’s support network, including family members, healthcare providers, behavioral health professionals, social service agencies, and legal representatives.

  • Facilitate communication among providers, organize case conferences, and utilize team expertise to support client care.

  • Create, maintain, and share as appropriate a dynamic self-care plan.

  • Enter required client, service, and assessment data accurately into designated systems for reporting and submission to Heartland Family Service.

  • Prepare, review, and transmit required documentation and data reports to Heartland Family Service according to established timelines and standards.

  • Serve as a liaison between Zion and Heartland Family Service to support effective communication, coordinated workflows, and shared accountability.

  • Work collaboratively with Zion staff and Heartland Family Service teams to improve care coordination, data sharing processes, and program effectiveness.

  • Participate in meetings, case consultations, or workgroups with Heartland Family Service as needed to support collaboration, quality improvement, and CCBHC alignment.

  • Identify opportunities to strengthen coordination, reduce duplication, and improve continuity of care between Zion and Heartland Family Service.

  • Strive to make connections between the agency and the larger community whenever possible to contribute to ongoing fundraising and relationship building efforts.

  • Essential functions of this job are to be performed on company physical work site or designated workspace.
