Key Responsibilities:

- Assist the Director of Care Transitions with coordinating home health services and hospice referrals

- Issue notices and communicate effectively with guests and their families

- Maintain accurate and organized filing systems for all documentation

- Ensure attention to detail in all tasks to support seamless care transitions

- Support administrative tasks related to care coordination and documentation

- Collaborate with healthcare providers and team members to facilitate smooth service delivery
