Job Description - Case Assistant

JOB SUMMARY:

Responsible for assistance in all programmatic areas, as well as case assistance for older adults, ages 60+, who are enrolled as OAC and NORC participants.

WHAT YOU’LL DO:

Provide case assistance, including initial intakes, benefits/entitlements, information/referrals, advocacy, immigration services, and other case assistance as needed
Writing comprehensive, accurate, and timely case notes
Data entry into DFTA computer software program
Completing and submitting monthly unit logs for case assistance and information/referral
Assist with promoting and implementation of all department programs, as needed
Responsible for transportation intakes and maintaining monthly unit service logs for participants
Responsible for compliance to DFTA standards, including unit mandates
Assist with review and entry of units, as needed
Updating client online profiles as required
All other projects assigned by Supervisor

CORE COMPETENCIES:

General case assistance, benefits and entitlements, work with older adults, tracking, reporting, program assistance, adhering to all required deadlines

MINIMUM REQUIREMENTS:

Experience working with constituents on a one-to-one basis; experience working with active adults a plus; Spanish speaking a plus

SALARY/HOURS:
Salary: $42,000

Full-Time / 40 Hours
