SUMMARY The Case Manager will serve and provide support to clients on a daily basis. This person should be focused and motivated, while also working with the facility management team to communicate or handle client-related issues. The Case Manager must demonstrate an acceptable level of maturity, good judgment, and emotional stability. Candidates must be willing to work a flexible schedule, including some weekend and overnight shifts.

DUTIES AND RESPONSIBILITIES

· Update NSP's as needed.

· Develop summaries for staff meetings based on the N&S

· Provide a summary of clients for all county meetings.

· Attend all staff meetings as scheduled by Program Directors and PA’s

· Plan and organize group sessions (M-F)

· Find and plan fun in-house activities for Clients.

· Transport to scheduled court appointments (as needed)

· Meet with each Client 1x1 for 20-30 min to discuss NSP goals, placement, or discharge.

· Develop a DAP note based on 1x1 and group interactions bi-monthly.

· Develop activity calendar monthly.

· Locate placements for clients.

· Preplacement interviews as needed, while completing the functional capability form and preplacement appraisals

· Review client referrals.

· Complete survey within 24-48hours of arrival (file under TX plan)

· Meet with County case managers and conservators as needed.

· Accurately manage any appointments and communicate to CMD and CRC

· Report accordingly/crisis response team.

· Attend weekly team/clinical/1x1 meetings.

· Needs and Service Plan completed within 72 hours, with signature from yourself (licensee) and the client (or conservator as needed)

· Answer all texts, acknowledging receipt of all messages.


Requirements

  

QUALIFICATIONS 

· College degree is preferred, but not required. 

· Must be at least 21 years of age

· PPD test for tuberculosis and satisfactory results  

· One (1) year of experience preferred. Understanding of psychiatric illness and its treatment, and knowledge of psychotropic medication.  

· CCL approved FBI and DOJ criminal background clearances. 

· Free from state sanctions  

· Valid driver license 

· CPR/First Aid/BLS certification Previous experience with population to be served and/or work in a similar program setting preferred.  

· Able to read and write clearly and accurately. 

· Act as a team member providing input and reporting observations and concerns regarding client care.  

· Demonstrate the ability to act as a part of a team and in an appropriate manner in a crisis.  

· Ability to work independently and be self-motivated in an often-ambiguous environment.

· Proven track record with maintaining strict confidentiality and appropriately work with sensitive information/documents.
