Job Description - Case Manager


  

PURPOSE: Provides support to the participants of the Cardinal Manning Center (CMC).

Key RESPONSIBILITIES:

General Tasks:

· Participate in problem solving, staff and program development with the guidance of supervisor.

· Provide intense case management services to assigned clients.

· Develop effective, trusting relationships with clients with a focus on facilitating their move towards independence. 

· Provide crises intervention, short-term and long-term case management services.

· Housing search and placement for clients including accompanying clients to housing appointments if needed.

· Provide information, referrals, and social services advocacy.

· Maintain accurate and timely documentation of client information.

· Network with agency and outside service providers, ensuring client access to appropriate services.

· In concert with supervisors assume responsibility for attending assigned community meetings. Meetings will be assigned based on the needs of the department.

· Support to weekly skid row seminar series.

· Participation in supervision, case management, and staff meetings.

· Community outreach and collaboration with other social service agencies.

· Other duties as assigned.

WORKING CONDITIONS & PHYSICAL WORK DEMANDS:

· Requires ability to sit at computer for long periods of time and move about on foot.

· Stoop, kneel, bend at the waist and reach on a daily basis.

· Requires standing and some travel time. Varied shifts including evening and weekend may be required.


Requirements

  

Qualifications: Bachelor’s degree in any human service-related field; experience with Microsoft office; experience with homeless population. Bilingual Spanish/English preferred.

Salary Description

$25- $28 Hourly
