Housing and Support Case Manager

Key Responsibilities:

- Assist participants with identifying and addressing their housing needs and setting achievable goals.

- Prepare, maintain, and organize all required documentation and records accurately and in a timely manner.

- Coordinate closely with other service providers, agencies, and community resources to ensure comprehensive support for participants.

- Develop rapport and build trusting relationships with program participants, demonstrating empathy and understanding.

- Uphold the agency’s philosophy and principles, including choice, empowerment, cultural sensitivity, and holistic support.

- Promote participant engagement through peer and natural supports, fostering a collaborative and respectful environment.

- Monitor and evaluate participant progress, adjusting support plans as needed to promote success.

- Ensure safety, accountability, and transparency in all interactions and documentation.

- Participate in team meetings, training, and ongoing professional development to stay aligned with best practices and agency policies.

Skills and Qualifications:

- Proven experience in case management, social work, or related fields preferred.

- Strong interpersonal and communication skills, with the ability to develop rapport and trust.

- Knowledge of housing resources, mental health principles, and community support systems.

- Ability to work independently and as part of a multidisciplinary team.

- Excellent organizational skills and attention to detail.

- Cultural competence and sensitivity to diverse backgrounds and experiences.

- Valid driver’s license and reliable transportation may be required depending on program needs.

Requirements

     

Minimum Qualifications:

Bachelor’s degree in social science OR associate degree   and two (2) years human services experience preferred; OR High School   Diploma/GED and four (4) years human services experience; valid driver’s   license.  PA State Police Background check. PA Childline.  FBI   Fingerprint Clearances.  E.O.E
